Please fill in or answer ALL the questions. If you don't know the answer to a question or it doesn't apply to you, please choose the "N/A" (Not Applicable) option or type "N/A" into the relevant box.

Some questions have Yes or No options. For all such questions, always click either Yes or No--don't leave it blank. Note that No also means Not Applicable. Some of these questions also have Past and Present options. Tick Past if the activity happened in the past. Tick Present if the activity is on-going. If the activity happened in the past and is still on-going, tick both Past and Present.

Some questions relate to family members and have several check boxes--tick all that apply. If the headings of these check boxes are abbreviated (e.g. UM, AF, etc.), hovering the mouse over such an abbreviation will display a popup help message explaining the abbreviation.

Each form has several sections. Use the and buttons to move from one section to another. Each time you move to a new section, all the changes you've made are automatically saved (you can also click the "Save" button to save any changes made). If you need to save the information and return to it later, click on the "Save and Logout " button. If you have finished filling in all the forms and wish to submit all the information to the Sorting Out Team for processing, click on the "Save and Submit" button.
